1. The online form should not be completed in isolation but should be completed in context of the full policy and the protocol for managing substance use related incidents. more
2. The name of young person is not required on the form but workers/managers may wish to include an identity reference code for service records only. more
3. The full policy and protocol clearly outline when there is a legal and safeguarding obligation to report the name of the young person to the Police or Social Care. more
